CONFIDENTIAL CITIZEN INCIDENT REPORT
The incident report form below is completely confidential. There is no
way to know who sent the message or where it came from, unless you choose to
enter your real name and/or email address. You must make some kind of
entry in the Name box in order to submit the report. It can be your real
name or a nickname, which could help us to better respond. Just enter
something. You do NOT have to enter your email address, but it will help
us in getting more info on the incident, if needed.
Please include the following:
- Address:
Be as accurate as you possibly can. If known, the actual address of
the incident is extremely helpful. Or the location in a block.
Or the corner intersection (NE, NW, SE, SW).
- Date and Time: Be specific. If multiple occurrences, give the
most recent first.
- Description: A brief description of activity (drug dealing, drug
use, prostitution, under age drinking, fights,
robbery, etc) and a description of the individuals involved (gender,
ethnicity, age, height, clothes, car, etc.) Is it an on-going
activity: ex; Friday nights 8-12, weekday afternoons at 3pm, etc.
